  • Symptom Intensity: 1 = minimal discomfort, 10 = extreme discomfort
  • Duration: Overall length of time you have been experiencing a symptom

  • Lost ability to hit high notes when singing 
         
  • morning headaches or tiredness
         
  • Pain with swallowing
         
  • Raspy voice
         
  • Raw, achy or strained feeling in your throat
         
  • Recurring chest superinfection
         
  • Red, swollen tonsils covered completely or in patches by pus
         
  • Redness of the tonsils and throat
         
  • Regurgitation of food
         
  • Repeatedly clearing of throat
         
  • Snoring
         
  • Sore throat
         
  • Sounds like you have a cold when you don’t
         
  • Talking required more effort
         
  • Tenderness in the glands of the neck (swollen lymph glands)
         
  • Wake up frequently in the middle of the night
         
  • White patches on the tonsils
